Job Description:
A company is looking for a Care Manager, Registered Nurse-
Remote.
Key Responsibilities
Engage telephonically with members, caregivers, and providers to
develop and implement comprehensive care plans
Collaborate with healthcare professionals to meet health care goals
and assess ongoing care needs
Coordinate patient education, referrals, and transitions of care
while maintaining documentation standards
Required Qualifications
Current multi-state compact Registered Nurse licensure; BSN
preferred
Minimum 3-5 years of clinical experience, with telephonic Case
Management experience preferred
Training in motivational interviewing preferred
Proficiency in electronic medical records and Microsoft Office
applications
Ability to work from a home office that meets HIPAA
requirements
