Job Description:
A company is looking for an Advanced Practice Provider (Nurse
Practitioner / Physician Assistant).
Key Responsibilities
Conduct comprehensive wellness exams in-home and virtually
Educate patients and families about healthcare programs and
treatment options
Coordinate care with primary care providers, specialists, and
ancillary services
Required Qualifications
Active and unrestricted RN and NP/PA license in the state of
practice
Board certification (NP: ANCC/AANP; PA: NCCPA)
Current and unrestricted DEA certificate
Minimum 2 years of direct patient care experience, preferably in
managed care or chronic disease management
Bilingual (English/Spanish) required
