starting at $40+/hour depending on experience!
- Days, 8 hr shifts
- 20 hours weekly in manager role, 20 hours direct care
nursing
- Rotating every 4th weekend
- Shift times flexible Job Summary The RN Case Manager, Long Term
Care (LTC) assesses, implements, and coordinates a plan of care for
residents in pertinent areas. This position assures medications and
treatments are administered to assigned residents, assists with
plan of care and interventions for effectiveness, and assesses for
accuracy of the MDS. Focuses on providing residents with
opportunities for independence and maintaining self-esteem,
privacy, and confidentiality. Assures physicians' orders are
processed and physicians are notified in a timely manner as needed
while observing and documenting observations and care given to
residents. Directs and assists with resident, family, and
significant other education in conjunction with care team. Seeks to
evaluate outcomes based on an integration among established
clinical, financial and utilization data. Functions in structured
and unstructured health care settings described as a geographic
and/or situational environment that may not have established
policies and procedures. Utilizes independent nursing judgment when
integrating health care. Has knowledge of and utilizes appropriate
age-specific structured care methodologies, such as
protocols/integrated clinical pathways/guidelines/standards of care
relating to the overall health care needs of the residents.
Interviews potential residents and assesses residents in compliance
with regulations, policy and procedure. Uses assessment information
for admission and continued stay in the long term care community.
Ensures all residents meet assisted living qualifications as
identified in the occupancy agreement, if applicable. Establishes a
service plan for each resident and evaluates on an on-going basis.
Able to work with growth and development needs of client
populations in clinical area. May work with clients in all age
groups. Ability to communicate with residents, family members and
others on the health care team. Coordinates, facilitates, and
negotiates with others. Monitors, evaluates and trends resident
responses utilizing structured care methodologies. Fiscally
responsible. Controls variances and duplication. Must demonstrate
independence, assertiveness and critical thinking when working with
residents and co-workers. Ability to document clearly. Ability to
solve problems. Self-motivated. Works with little direction.
Ability to incorporate teaching/learning principles and adapt
teaching to age group. Ability to facilitate groups and implement
projects when necessary. Must be able to establish priorities, have
strong flexibility and organizational skills. Must be knowledgeable
about reimbursement for services provided. Will be required to work
primarily day hours, scheduled weekends, and occasional evenings.
May be exposed to communicable or infectious disease, hazardous
materials and injury from performance of assigned duties. Is
subject to multiple sensory and environmental stressors.
Qualifications Bachelor's degree in nursing preferred. Graduate
from a nationally accredited nursing program preferred, including,
but not limited to, Commission on Collegiate Nursing Education
(CCNE), Accreditation Commission for Education in Nursing (ACEN),
and National League for Nursing Commission for Nursing Education
Accreditation (NLN CNEA). Minimum of one to two years of
professional nursing experience in a clinic care setting preferred.
In-depth knowledge of specialty area preferred. Currently holds an
unencumbered RN license with the State Board of Nursing where the
practice of nursing is occurring and/or possess multistate
licensure if in a Nurse Licensure Compact (NLC) state. Obtains and
subsequently maintains required department specific competencies
