Job Description:
A company is looking for a Health Coach to support patients
living with dementia and their caregivers.
Key Responsibilities
Serve as the primary point of contact for patients and families
enrolled in the Care Ecosystem program
Collaborate with healthcare providers to improve patient health and
well-being
Provide education and resources, including behavioral management
and advanced care planning support
Required Qualifications, Training, and Education
Bachelor's Degree preferred
Valid Driver's License required; Health Coach Certification
preferred
Two years of experience in a social or healthcare setting or
equivalent education preferred
Bilingual and local community knowledge preferred
Proficient in computer use and knowledge of medical terminology
preferred
