Job Description:
A company is looking for a Chronic Care Management Enrollment
Specialist to enroll patients in their care management program.
Key Responsibilities
Initiate outbound calls to eligible patients to introduce and
explain the CCM program
Educate patients on the benefits of CCM and verify their
eligibility
Manage patient interactions and maintain accurate records of
enrollment status
Required Qualifications
Experience in customer service, particularly with elderly or
chronically ill patients
Goal-oriented mindset with the ability to meet enrollment
targets
Ability to work independently in a remote environment
Empathy and patience when addressing patient concerns
Familiarity with healthcare programs and guidelines
